First 5 San Bernardino is offering the following Investment Opportunities to the Community:
Event Sponsorship Policy and Application

Sponsorship requests are accepted by First 5 San Bernardino on an on-going basis. The maximum amount of sponsorship available is $10,000.00 per organization per fiscal year. Effective July 1, 2010: If approved, your event will receive up to 50% of the requested funds within 90 days of this application process. Upon conclusion of event you must submit an Outcome Report within 30 days. Once the review of the Outcome Report is completed, the remaining award will be determined based on the contracted and authorized expenditures. Review of Outcome Report is done within 7-14 business days of a completed submission.

In an effort to increase community awareness and further improve the lives of children prenatal through age five in San Bernardino County, First 5 San Bernardino may participate or sponsor events and activities that are consistent with the First 5 San Bernardino Strategic Plan and the Sponsorship Policy.

Please Note: The First 5 San Bernardino Strategic Plan and our Sponsorship Policy will serve as a reference when applying for a sponsorhip. The Strategic Plan and Sponsorship Policy are available on the website or at the First 5 Offices in downtown San Bernardino.

An Independent Review Panel reviews the Sponsorship Applications and scores them according to how well the applications relate to the First 5 San Bernardino Strategic Plan and the Sponsorship Policy.

Phase 1: Applying

Download the following documents to apply for a First 5 San Bernardino Sponsorship. All documents play an important part to the outcome of your sponsorship request. Please read documents and questions throughly and call First 5 San Bernardino sponsorship specialist, Rosy Nichols at (909) 386-7706 for further assistance with the application process.

  • Sponsorship Application  - will ask about your event. The more information you provide about the benefits and activities for children 0-5 or their families - the better!
  • Budget Template For Your Event  - an Excel document, provided by First 5, designed to make submitting your budget for the event simpler for us to review. Please be specific! No miscellaneous, general categories! Review the Strategic Plan and Policy before committing any part of your budget to be sure it relates appropriately.
  • Strategic Plan - use this tool to create a great event and budget!
  • Policy, The Policy is important in making sure your budget is appropriate. This tool is like the constitution for the application - PLEASE READ IT!
  • Request for New Vendor Code form - the address you provide in this form is where the check will be sent.
  • W-9 form - this form is to report the funds to the IRS.
  • Media Guidelines - describes in detail how the "First 5 San Bernardino" name and logo can and cannot be used. You will be ask to provide copies of all the marketing you do for the event and are expected to use the First 5 logo in your marketing and promotional items if possible.
Phase 2: Outcome Report
 
Outcome Reports  - Per the Policy, you are required to submit this report 30 days or sooner from your event. The Outcome Report is NOT complete unless it’s submitted with:

  • The "TEMPLATE FOR SUBMITTING RECEIPTS"  Excel document, provided by First 5, clearly showing what the sponsorship was spent on.

  • ALL the original receipts related to the sponsorship expenditures.

  • If, applicable, a check or money order of the unspent monies left from the sponsorhip.
Frequently Asked Questions
  1. What publicity MUST First 5 San Bernardino receive from us, in exchange for sponsoring our event?

    The First 5 Sponsorship must be used to acknowledge First 5 San Bernardino on all public education and outreach materials produced for the sponsored event, either in electronic or printed format. For example, if you decide to print flyers to advertise your event, it MUST have the First 5 logo and you MUST send us a copy of that flyer for our records, either through email, mailed, or a faxed hardcopy. These materials include, but are not limited to: brochures, flyers, posters, Save-the-Date announcements, Registration and program materials, TV/Radio, PSA's, newspaper and newsletter print advertisements and event logo merchandise.

    The wording of the attribution should be as follows: "In partnership with First 5 San Bernardino". Where appropriate, sponsorship recipient shall use the First 5 San Bernardino logo in accordance with First 5 San Bernardino Attribution Standards. Promotional materials for the event must be submitted to First 5 San Bernardino in electronic mail form (PDF and MS WORD format).

  2. cWhat requirements does First 5 San Bernardino have in terms of representation at an event?

    First 5 San Bernardino shall receive complimentary registration to the sponsored event for select staff members. Use of a display or exhibit booth at the event will be available free of charge to First 5 San Bernardino. If the event will have a presentation or forum, they must invite First 5 San Bernardino representatives to participate. When appropriate, First 5 San Bernardino may display logo banner or sign at the event.

  3. What happens when I turn in an application?

    The application is reviewed internally by First 5 staff to make sure that it is complete. Once the application is deemed complete, it is then, sent to an Independent Review Panel for scoring. If the average of all the scores "passes" then the applicant is notified and itʼs processed for payment.

  4. How do we spend the First 5 sponsorship monies if we are approved?

    Your expenses should only reflect the approved expenses presented in the budget template portion of the Sponsorship application.

  5. What are the next steps after my sponsored event is complete?

    Please submit an outcome and expense report. (Templates are provided for your convience.) Reports must be received to First 5 San Bernardino within 30 days of the end of the event for future consideration.

  6. What if I do not turn in the Outcome Report after my event?

    In order to be considered for future sponsorships, the outcome report must be submitted within 30 days.

  7. What should I do with unspent sponsorship funds?

    According to our Policy the funds must be returned as a check or money order within 30 days after the completion of your event, along with the Outcome Report. Checks should be made out to "First 5 San Bernardino" and sent to:
